APPLICATION FORM



District 202 has many outstanding alumni who have used their education and experience to become successful, positive contributors to society. The Foundation for Excellence Alumni Hall of Fame has been established to recognize and showcase outstanding graduates as part of the Foundation's work to promote excellence and achievement for learners in District 202. The Alumni Hall of Fame is displayed on the Foundation's website.

Nominations

The Foundation is accepting nominations for the Foundation for Excellence Alumni Hall of Fame in the following categories:

  • Arts and Entertainment
  • Business and Commerce
  • Community Service and Volunteerism
  • Education and Humanities
  • Government and Military
  • Medicine and Health
  • Philanthropy and Non-Profit
  • Science and Technology
  • Sports and Recreation

Criteria to be Considered

  • A nominee has graduated from a District 202 high school, or the Plainfield High School which preceded the formation of District 202, at least twenty years prior to nomination.
  • A nominee has significant accomplishments and/or community service that have been recognized as outstanding through the receipt of awards, honors, or other forms of distinction,
  • A nominee has made a profound and positive international, national, regional or local impact on their chosen field or community.
  • Posthumous nominations will be accepted.

Selection of Inductees

The selection committee is comprised of Board of Trustees of the Plainfield School District 202 Foundation for Excellence.


Procedure for Nominations

  • Nominations may be made by alumni, current or former faculty and staff, administrators, and community members of District 202.
  • Nominations need to be submitted by January 20, 2023. Each of the attached nomination forms should be fully completed and submitted as outlined on the form.
  • Nominations will be kept on file and reconsidered for the next two years following submission. After three years, a candidate may be re-nominated.

Induction

New Alumni Hall of Fame members will be inducted at the annual Foundation Dinner in April.
